A Mobile Health Project Committed to Caring for the Health Needs of Our Community. The Community Care Team can assist people in their own homes. We will work together with the healthcare system to be sure our community members get the best care possible!

WHAT WE DO

HEALTH ASSESSMENTS 

  • Blood Pressure screening and monitoring

  • Chronic disease monitoring and education

  • Medication set-up / compliance

REFERRALS TO DPHHS

  • Social Services

  • Mental Health

  • Substance use disorders

TRANSPORTATION

  • Provide rides to and from local clinics

  • Transport home from local hospitals

PREVENTION AND PUBLIC HEALTH 

  • CPR and First Aid classes

  • Facilitate Diabetes Education

  • Car seat checks

  • Public Health Initiatives (i.e. seat belts. etc.) 

  • Welfare checks

  • Informal Counseling

  • Connecting to Community Resources

HOME SAFETY CHECKS

  • Physical environment assessment

  • Identify hazards

  • Modification recommendations

HOSPITAL CHARGE AND FOLLOW-UP CARE

  • Wound care, dressing changes

  • ECG analysis
